ED drugs are among the most counterfeited pharmaceuticals in the world. Here's how to tell a legitimate telehealth service from a site that will mail you an unknown powder.

There is a legitimate online market for ED medication, and there is a large illegitimate one wrapped in similar branding. The distinguishing question is simple and almost never wrong.
What legitimate services look like
- A real medical intake, reviewed by a clinician licensed in your state, before anything ships
- A named, licensed pharmacy doing the dispensing
- A verifiable US business address and a working way to contact a human
- Clear pricing stated before you enter payment details
- Refusal to sell you a contraindicated drug — a good service will decline if you report nitrate use
Red flags
- “No prescription needed” — the single clearest signal
- Prices far below even generic cash pricing — generics are cheap, but not free
- Payment by wire transfer, cryptocurrency, or gift card
- No physical address, or one that doesn't resolve to a real place
- Spam email or text as the acquisition channel
- Shipping from overseas to bypass US prescription requirements

Why counterfeits are genuinely dangerous
ED medication is one of the most counterfeited drug categories globally, because demand is high, embarrassment suppresses complaints, and buyers self-select into unofficial channels.
Seized counterfeits have been found to contain the wrong dose, no active ingredient at all, entirely different drugs, or industrial contaminants. The failure mode that actually kills people is subtler than a fake pill, though: a product sold as “herbal” with undisclosed sildenafil in it. Someone taking nitrates for heart disease avoids Viagra deliberately — and has no way to know the supplement they bought contains the same molecule.
